The Disabilities, Research, Education, Advocacy, Movement (DREAM) organization has developed a survey to assess the quality of life of children and adolescents with intellectual disabilities. This survey takes about 10 minutes to complete and all responses are anonymous. In order to be eligible, you must be a parent/caregiver of a child and/or adolescent between the ages of 5-17 with an intellectual disability.
